South Dakota Medical Cannabis Card

1.) Debilitating Medical Condition

You may qualify if you have a “Debilitating Medical Condition”. Most people qualify for “Severe, debilitating pain”.

  • (a)    A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following: cachexia or wasting syndrome; severe, debilitating pain; severe nausea, except nausea associated with pregnancy; seizures; or severe and persistent muscle spasms;

    (b)    Epilepsy and seizures;

    (c)    Crohn's disease;

    (d)    Acquired immune deficiency syndrome or positive status for human immunodeficiency virus;

    (e)    Amyotrophic lateral sclerosis;

    (f)    Multiple sclerosis;

    (g)    Cancer or its treatment, if associated with severe or chronic pain, nausea or severe vomiting, or cachexia or severe wasting; or

    (h)    Post-traumatic stress disorder

2.) In-Person Medical Evaluation

Attend consultation and evaluation with a provider to see if you qualify for certification. At REMEDY 605, a $200 fee will be required for initial evaluation ($150 fee for recertification).

3.) South Dakota Medical Cannabis Program

Create an online account at South Dakota Cannabis Program website. There will be an additional $75 fee to the state of South Dakota ($20 if you qualify as low-income).

4.) Wait for Approval

Wait 1-2 weeks for card to be approved and mailed to your listed address.

5.) Return for annual recertification

You will need an annual in-person evaluation to maintain card certification. Recertification is essentially the same process as initial certification.

  • The age to qualify is 18. If you are under the age of 18, you must be accompanied by your legal guardian/caregiver at the time of evaluation. There are restrictions on the types of medical cannabis available to patients under the age of 21.

  • Unfortunately, not at this time.

  • PSW takes privacy and health information security seriously. The online registry system for patients meets all state and federal standards for confidentiality, accessibility, and information security.

  • Yes, so long as the designated caregiver:

    -Is at least twenty-one years of age;

    -Has agreed to assist with a qualifying patient's medical use of cannabis

    -Has not been convicted of a disqualifying felony offense; and

    -Assists no more than five qualifying patients with the medical use of cannabis, unless the designated caregiver's qualifying patients each reside in or are admitted to a health care facility or residential care facility where the designated caregiver is employed.

  • An in-person evaluation by a certified medical provider is required by the law. This includes initial evaluation and recertification.

  • Full Payment - we accept card and cash

  • Although it is not required, it is recommended that you bring all medical records that support your qualifying medical condition.

  • After in-person medical evaluation, on average, it can take up to 1-2 weeks to obtain your cannabis card in the mail. You must wait to receive your card in the mail prior to purchasing medical cannabis at a dispensary.

  • The South Dakota Medical Cannabis Program or PSW cannot provide legal advice to qualifying patients as to the impact of medical cannabis use. Qualifying patients or others with legal questions should consult with their own legal counsel.

  • As part of the application process to obtain a registry identification card for the South Dakota Medical Cannabis Program, applicants may also apply to cultivate no more than two flowering cannabis plants and two cannabis plants that are not flowering plants in their home.

  • Yes it does, but you must bring medical documents proving that you have a prior diagnosis by a qualified mental health provider.
